I have done this procedure , so this is update information and you can follow it if you may need English version Certificate and Mark sheet.
The procedure are following.
1. Download the form from the link
Application form
2. Fill up the form with no overwriting with perfect Spelling and address.
3. Go to your School/Collage , verified picture with Head Sir / Principle.
4. Go to Dhaka Education Board Office ( Bokshi Bazar )
5. Make a Bank draft of 500/= Tk from Sonali Bank Board office branch( Bahvan 2), you have to pay 23/= tk as per commission of Bank. So you shall need 523/= taka to Bank draft. Form are available in Bank. It is a easy task , just need 5 minutes.
6. Deposit the Documents in the counter of beside or information office . It may also need another 5 minutes.
7. This counter will received the Bank draft , given back your document and instruct you to go Bhavan - 4 , 4th floor , counter 6.
8.Go to Bhavan-4, 4 th floor , counter 6 , submit here and take a red part of money receipt for receiving the Certificate/ Mark sheet .
If you lost paper you have to make a GD in local Thana , then make a daily newspaper advertizement and the start from serial 1. If you are busy there are some people near Board office who can done this job with 500 tk
